Q: HOW LONG DOES THE ENROLLMENT INTO AN ADULT FAMILY HOME TAKE?

A: If all the information is complete on the enrollment forms and the required documents are received (discharge papers from hospital, Power of Attorney papers, medication list, nurse assessment, etc.), then the process goes fairly quickly.

Q: CAN MY LOVED ONE KEEP THEIR CURRENT PRIMARY CARE PROVIDER?

A: All of our residents keep their Primary Care Provider. They can choose to use our on-call visiting physician if it they are not easily transported out to doctor’s appointments.

Q: HOW DOES THE SERVICE WORK?

A: MY AFH works to provide 24/7 care for your loved one. This includes housing, meals, medication management, healthcare needs, physical activities, hair, nail, and podiatry needs and many of the other basic in-home needs. We have caregivers on site 24 hours a day, seven days a week.

Q: DO YOU COORDINATE CARE WITH HOSPICE?

A: Yes. MY AFH works closely with the registered nurses and other ancillary staff from Hospice to help provide a well-rounded continuum of care for our residents.

Q: WHAT IF MY LOVED ONE NEEDS SPECIALIZED CARE?

A: MY AFH has specialized homes to accommodate all levels of care. We have homes that specialize in behavioral management and bariatric care. These homes are designed to help keep costs down for our residents. We also specialize in diabetes, feeding tubes, medication management, and all forms of dementia and mental health.

Q: WHAT IF MY LOVED ONE RUNS OUT OF FINANCIAL RESOURCES?

A: At Tranquil Gardens AFH, we honor the original contract to provide care as long as a resident is with us. This includes a 36-month private pay policy (or any other agreed upon payment structure). There is no need to worry about needing to move your loved one again as I do work with Medicaid after 36 months.

Q: WHAT IS YOUR VISITATION POLICY?

A: We follow the guidelines set forth by governing entities, which may change with the COVID-19 pandemic. We are considered a healthcare facility and require all visitors and vendors to wear masks in the home. Visitors are welcomed to the home during open visitation hours and visits must occur in the Resident’s personal room. The maximum number of guests are 2 at a time

Q: WHAT DOES TRANQUIL GARDENS AFH PROVIDE?

1. Safety and Security: We are designed with safety in mind, and qualified personnel are on duty to address any need or situation of a resident.

2. Personal Living Experience: Residents interact with other residents and can establish their own comfortable routine. Community involvement means friends and friends keep you feeling young and happy. An active social life is fundamental to health and well-being.

3. Lifestyle Enhancement: Activities encourage physical and mental wellness. Advanced nutrition services combined with contemporary wellness programs greatly increase overall physical and mental health.

4. Assistance and Support: Whether a resident needs help with daily living activities or simply feel isolated and alone, our fully trained staff will be there for assistance. Each day residents may be greeted by a caregiver who will aid with daily living skill needs of bathing, dressing, grooming and medication monitoring.

Q: WHAT ARE SOME ACTIVITIES YOU PROVIDE?

1. Every Thursday we have a certified music therapist come to the home for a group session.

2. Every other Saturday, we have a Cabaret performance where a local Broadway performer does a one-hour song and dance show.

3. Raised bed Gardening – We take the Residents onto the back deck to help plant and care for our garden.
